Case Management
After the initial evaluation conducted during our weekly events, we develop individualized case management plans tailored to each person’s unique circumstances. Our navigators work closely with each individual to understand their personal story, assess their immediate priorities, and establish a comprehensive roadmap that addresses both urgent needs and long-term goals. This personalized approach ensures that every participant receives the support they need while fostering a sense of empowerment and autonomy as they navigate their new environment.
AFL has implemented a streamlined and efficient case management and referral system designed to address the unique challenges faced by newly arrived migrants and asylum seekers. This system simplifies access to essential services by guiding individuals through government agencies—such as applying for public benefits—and connecting them with trusted nonprofit organizations that offer specialized support to meet critical needs.
We provide culturally and linguistically appropriate guidance to help participants access a wide range of essential local services, including healthcare, mental health support, housing assistance, legal aid, financial literacy programs, and English as a Second Language (ESL) classes. In addition to these external resources, participants also benefit from referrals to AFL’s internal programs, such as Immigration Processes, Workforce Development, Financial Education, and Socio-emotional Support. These programs address the systemic barriers migrants face by providing the tools and resources necessary for successful integration.
Additionally, our Case Navigators collaborate with external organizations to connect asylum seekers with key services, such as enrolling in health insurance through MetroPlus, accessing affordable housing through Fundavenyc, and receiving OSHA training with Mixteca. By leveraging these strategic partnerships, we ensure that participants have access to a continuum of care that enables them to rebuild their lives with dignity, stability, and security in their new community.
