AID FOR AIDS’ Case Management program was born as a core component of the Access to Treatment program. From our earliest years, we understood that delivering medications was not enough — people needed clinical support, guidance, and follow-up to sustain their health over time.
As AFA grew institutionally and territorially, the model evolved into a comprehensive support system that today connects health, rights, and social integration.
From Clinical Follow-Up to Comprehensive Integration
Initially, case management focused on clinical follow-up for people receiving treatment, therapeutic adherence support, coordination with treating physicians, and oversight of treatment continuity.
With the opening of the New York office and the development of the support program for immigrants living with HIV, the model expanded to respond to the multiple needs people face when entering a new health system. Today, case management includes health system navigation, coordination of medical appointments, mental health support, psychosocial counseling, basic legal guidance, linkage to housing and social services, and education on rights and self-care.
Reach and Community Linkage
Case management is activated through community outreach strategies that identify people outside the health system. AFA develops prevention and testing events, work with community-based organizations, identification of people in highly vulnerable situations, and immediate linkage to care pathways.
Community outreach is the gateway to comprehensive support.
As part of its evolution, AFA has developed technological tools to strengthen follow-up in contexts of human mobility. Tu Salud Digital, currently in pilot phase and progressive expansion, enables remote support, clinical follow-up, and service coordination along migration corridors. This tool complements the human support of the case management team.
